The procedure to get a new card in the event of loss of the card of the Mediclaim Policy of the Employee appointed after 01.01.2004 is as follows –
For this you will first get a blank card of Mediclaim Policy from the office of General Insurance and Provident Fund Department. HereAfter, you will complete the card and paste the photos of all the family members on it. After doing this, you will get your Mediclaim Policy Card certified with DDO just like the new Policy Card.
After this, your old policy card has been lost, make an affidavit to this effect and get it certified by notary. It keeps you attached together. After that, submit the new certified policy card duplicate card with the affidavit to the office of General Insurance and Provident Fund Department along with the application form. After this, after 4 to 5 days you will get your new duplicate card from GPF office.
